DATA PRIVACY CONSENT

I hereby acknowledge and authorize Panabo Multipurpose Cooperative:

1. To use my information for the processing of transactions, including but not limited to membership application, loans, savings, mortuary services, and other forms of assistance. My information may also be used for reporting to the Management, Board of Directors, and regulatory bodies; establishing and maintaining member information systems; conducting analyses for product or service enhancements; making announcements; promoting events, programs, and activities related to membership care and growth;;
2. To collect, record, organize, update or modify, retrieve, consult, use, consolidate, block, erase or destruct my personal data as part of my information;
3. For the regular submission and disclosure of my basic credit data (as defined under Republic Act No. 9510 and its Implementing Rules and Regulations) to the Credit Information Corporation (CIC)as well as any updates or corrections thereof;
4. For the sharing of my basic credit data with other lenders authorized by the CIC, and credit reporting agencies accredited by the CIC

I hereby affirm my right to be informed, object to processing, access and rectify, suspend or withdraw my personal data, and be indemnified in case of damages pursuant to the provisions of the Republic Act No. 10173 of the Philippines, Data Privacy Act of 2012 and its corresponding Implementing Rules and Regulations
